For more details regarding quality of photo, <a href=\"\/passport\/?page_id=773\">click<\/a> here; and<\/li>\n<li>Additional documents as applicable.<\/li>\n<\/ul>\n<p><strong>Application for a Mauritius Passport in replacement of a lost\/stolen\/damaged valid passport<\/strong><\/p>\n<p><strong>DOCUMENTS TO BE SUBMITTED.<\/strong><\/p>\n<ul>\n<li>Application Form ( PF136) duly filled and signed by applicant and parents\/Guardian (where applicable);<\/li>\n<li>Birth Certificate of applicant;<\/li>\n<li>Birth Certificate of parents (where applicable);<\/li>\n<li>Mauritius National Identity Card of applicant;<\/li>\n<li>Mauritius National Identity Card of parents (where applicable);<\/li>\n<li>Marriage Certificate of applicant (where applicable);<\/li>\n<li>Marriage Certificate of parents (where applicable);<\/li>\n<li>Divorce Decree of applicant (where applicable);<\/li>\n<li>Certificate of Foreign Citizenship or Residence Permit Abroad of applicant (where applicable);<\/li>\n<li>Certificate of Foreign Citizenship or Residence Permit of parent(s) (where applicable);<\/li>\n<li>Certificate of Mauritius Citizenship of parent(s) (where applicable);<\/li>\n<li>Certificate of Mauritius Citizenship in case applicant acquired nationality through Registration \/ Naturalization \/ Adoption (where applicable);<\/li>\n<li>Two (2) recent identical photos (passport size) of applicant taken against a light background. For more details regarding standard of photo, <a href=\"\/passport\/?page_id=773\">click<\/a> here;<\/li>\n<li>Certificate from Police in case of loss\/stolen passport;<\/li>\n<li>A letter to give a brief report of how, where and when the passport was lost\/stolen\/damaged;<\/li>\n<li>The original Police report for the lost passport should include reference, signature and seal\/stamp of the Police Station<span style=\"text-decoration: line-through;\">)<\/span> or a report made by the Embassy or High Commission of Mauritius;&nbsp;&nbsp;&nbsp;<\/li>\n<li>Additional documents as applicable<\/li>\n<\/ul>\n<p><strong>Where <\/strong><strong>Mauritius High Commission \/ Embassy or Consulate is <\/strong><strong>not<\/strong><strong> available, application may be submitted through a close relative in Mauritius.<\/strong><\/p>\n<p><strong>DOCUMENTS TO BE SUBMITTED.<\/strong><\/p>\n<p><strong>In addition to the<\/strong> <strong>documents mentioned above:-<\/strong><\/p>\n<ul>\n<li>A letter from applicant authorizing a close relative in Mauritius to submit the application at our Office and to collect the passport once ready;<\/li>\n<li>In case applicant is less than 18 years,<\/li>\n<li>Consent of parents on the Application Form should be certified by:-<\/li>\n<li>the Police with the name and signature of the Police Officer and seal\/stamp of the Police Station; or<\/li>\n<li>Attorney-at-Laws or Barristers or Notaries with name and signature of the person and the official seal\/stamp of the Office.<\/li>\n<\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<\/div>\n\t\t","protected":false},"excerpt":{"rendered":"<p>All applications have to be submitted through the Mauritius High Commission \/ Embassy or Consulate.
