FILLING THE FORM
Before filling the form, kindly follow the “NOTES” on the front page of the form.
Complete all relevant sections of the Form in CAPITAL LETTERS with a BLACK ink pen.
Consent of parents / Legal guardian at section (4A) and Certification of Consent at Section (4B)
- Where applicant is a minor (under 18 years), both parents (where applicable) should sign in presence of a police officer in a police station or at the Passport and Immigration Office
If parent(s) is/are abroad, consent may be given in the presence of:-
- Either a Police Officer in a police station ,
- An Official of a Mauritius Embassy/ Consulate,
- A Notary or a Solicitor, from the country of residence of the applicant.
Where certification of consent of parent(s) could not be done as per above
- Parents(s) should sign at section (4A),
- Parent(s) should give consent in form of a letter sent to the Passport and Immigration Office by registered post authorizing his/her Child to be issued with a Mauritius Passport,
- Letter should include exact name(s) of parent(s) and applicant as documents submitted,
- Signature(s) of parent(s) must be similar as section (4A) of application form and documents submitted.
Police Certificate at Section (5)
- Please refer to Notes 4 on the Application Form
- Section 5 is not applicable if applicant is abroad.
Specimen Signature at Section (7)
- Specimen signature or right thumbprint of applicant to be shown on the passport should be done with a bold black ink pen of size 0.6 mm, within the inner line of the box. An example as hereunder:-
Declaration of Applicant at Section (8)
- It is mandatory for applicant to sign or affix his/her right thumb print on the application form at section (8).
- For Minor– Either parent or Legal guardian should sign the declaration at section (8) of the application form.

All Citizens of Mauritius, either by

are eligible for a Mauritius Passport, provided that they satisfy all conditions as prescribed by law.
Application for a Mauritius Passport – FEES YOU WOULD BE REQUIRED TO PAY
New Passport
- Adult & Young Person – Rs 700
- Minor (of age below 16 years) – Rs 400
- Senior Citizen ( of age 55 years and above) – Rs 400
- Disable person (To produce a certificate from the Ministry of Social Security) – Rs 400
Endorsement of a passport Rs 100
Certificate of Identity / Travel Document Rs 300
New Passport in replacement of a valid Loss/Damage Passport
- First loss/damage – Rs 5000
- Second loss/damage – Rs 15000
- On every subsequent loss / damage : An additional and cumulative fee of – Rs 15000
The photos must be
- Identical;
- Recent (taken within the last six month);
- 35-40 millimetres (mm) wide x 45-50 millimetres (mm) high. (please do not trim your photos to meet this condition);
- Taken against a light background so that your features are clearly distinguishable against the background;
- Printed on low-gloss, plain white photo-quality paper (with no watermarks, embossing or printing on the back);
- Undamaged, for example, by creases from paperclips;
- Of you on your own (no toys, dummies or other people visible);
- A close-up of your head and shoulders so that your head, from the bottom of your chin to your crown is between 25mm and 34mm high; and clear of writing on the front and back.
The photos must also:
- Be in sharp focus and clear;
- Have a strong definition between the face and background; and
- Be printed professionally. Photos printed at home are not likely to be of an acceptable quality
The photos must show
- No shadows;
- You facing forwards, looking straight towards the camera;
- A neutral expression, with your mouth closed;
- Your eyes open and clearly visible (with no sunglasses or heavily tinted glasses, and no hair across your eyes);
- No reflection or glare on your glasses, and the frames should not cover your eyes (you may find it easier to remove your glasses);
- Your full head, without any head covering, unless it is worn for religious beliefs or medical reasons;
- Nothing covering your face. Please make sure nothing covers the outline of your eyes, nose or mouth.
Photos of children 5 years and under
The above-mentioned Standards for Passport Photos apply to photos of babies and young children too.
The key ones are:
- The baby or child should be against a light background so that their features are clearly distinguishable against the background.
- The baby or child should be on its own (no toys, dummies or other people visible).
- The full face should be seen.
- Both eyes should be open.
